Arizona Neurorehab provides social work support to help individuals and families navigate life after brain or spinal cord injuries. Our team assists clients in accessing community resources, coordinating care, and addressing personal challenges during recovery.

Education in Community Resources
We help clients navigate the complex network of community supports. This includes:
- Educating clients about available public benefits and services
- Assisting with referrals and application processes for:
- Cash assistance programs
- Health and behavioral health insurance
- Subsidized housing and childcare through the Department of Economic Security
- Social Security Disability Insurance (SSDI) and Supplemental Security Income (SSI) benefits
Our goal is to ensure that individuals and families receive the support they need to achieve greater stability and independence.
Case Management Services
Our case management approach is personalized, collaborative, and focused on long-term success. Services include:
- Developing individualized treatment plans to support the client’s highest level of functioning
- Coordinating with external case managers, payee sources, and community providers
- Maintaining ongoing communication with physicians and the interdisciplinary treatment team to align care with projected outcomes
